III. HOW TO REPORT A DEATH

When a person dies under any of the aforementioned circumstances, notify the Coroner. In order to contact the Coroner, call the Ashland County Coroner's Office at (419-282-6159); if no answer, follow pager instructions if you've been given them; if no response, call (419-289-0491) and request the Coroner.

A. When a call is made to report a death, it is requested that the following information be obtained and given at the time of the call.

1. Name and address of the deceased.
2. Age and date of birth.
3. Sex and race.
4. Social Security number.
5. Marital status.
6. Next-of-kin, name, address, phone number.
7. Place and manner of occurrence.
8. Date and time of occurrence.
9. Date and estimated time of death.
10. Name of person or observer relating signs of death.
11. Name of person reporting death.
12. Name of family doctor or treating physician.
13. Present location of the body.
14. Anything suspicious or unusual associated with the death.
15. Any other information which might be helpful.

A. Reporting by Law Enforcement, EMS and/or Hospital Personnel

Whichever of the above is first on the scene of a deceased and makes the call to the Coroner's Office, are to relate all the above information and fill out the coroner's information sheet. All the above information will be found on the reporting form making it easier to remember the information to be reported to the coroner. This information along with the "squad run report" and any law enforcement report are to be sent, faxed or brought to the Coroner's Office as soon as possible. In cases reported only, the coroner's office needs the information report along with the squad run, as soon as possible, so that if any investigation is necessary, it may be done before the disposition of the deceased is completed.
